Generally speaking, it depends.
If you request an autograph then you'll get a presigned photo which has the dedication done by his PA.
If you send a photo to be signed, you may get an authentic dedication, you may not. Depends how busy he is (they may do the personalisations for him and then he signs the stack in one shot).
It's easy to tell which you have though, as the formation of the letters are rather different.

Some say that it is his handwriting doing all the dedications, but IP dedications match those such as the VV one I posted, so it's unlikely that it's his handwriting.
