IMDb.com is not like wiki. People can edit things posted on Wiki, while on IMDb people cannot. People can send in submissions to those working at IMDb regarding films and such, but the people at IMDb more often than not checks the information sent into them before it gets posted.

Definitely the first address. You should always include the actor's name and the film title on the address whenever writing via venue, even though the postal service reads the address from bottom to top. Its just easier for the person who picks it up at the place to pass it along. {up}
