Preschool/Pre-K
Usually the year before entering elementary school, though there are some programs that begin even earlier than that.
Elementary School
Kindergarten: 5-6
1st Grade: 6-7
2nd Grade: 7-8
3rd Grade: 8-9
4th Grade: 9-10
5th Grade: 10-11
Middle/Intermediate/Junior High School
6th Grade: 11-12
7th Grade: 12-13
8th Grade: 13-14
High School
9th Grade (Freshman): 14-15
10th Grade (Sophomore): 15-16
11th Grade (Junior): 16-17
12th Grade (Senior): 17-18
College/University
Usually the four years directly after high school (ages 18 to 22/23), following the same ranking (freshman, sophomore, junior, senior). However, because college is "optional" and usually doesn't have much in terms of age limitation, some people may choose to put it off for a year, not go at all, or attend much later in life.
Additionally, in some areas where the number of schools are limited, there may not be a middle school and those grades (6-8) are instead covered either by an elementary school, high school or both.
