On 11-09-09 I sent Jerry a LOR, SASE, and a photo and DVD cover to:

Orpheum Theatre
910 Hennepin Ave
Minneapolis, MN 55403

I received my items signed by Jerry on 09-18-09 (he signed the reverse side of the DVD cover which is a half-toned photo of Seinfeld's living room. At First I thought it was a mistake, but the more I look at it the more I like it.
