When I usually have them, I just put the agent's name under the company or whatever...like this

celeb's name
c/o wherever company
agent's name
...

Or if there isn't a company name, I would do it like you said.

celeb's name
c/o or attn: agent's name

Okay, so let's say you are writing to someone and it was their agent or publicist. Would you write:

Celeb's Name
c/o Mr. Example
.....
.....

Mr. Agent
c/o Celeb's name

or

Celeb's name
Attention: Mrs. Publicist
