You could write something like: Dear NAME - Stage Manager of SHOW NAME.

2) What do you say in your letter? Do you say the same as if you were writing to the actor/actress?
Yes, you should say the same as what you would write in a normal letter. Why your a fan, what part of the show you liked, what actors/actresses you like in the show etc.

Hi all.

I am planning on writing a letter to the stage manager for a few shows on Broadway and a few questions arose.

1) Who do you address? (dear ?)
2) What do you say in your letter? Do you say the same as if you were writing to the actor/actress?
