First try to find an address that has been successful in the past. Research the feedback for the person(s) that you are interested in getting an autograph from. Check the address database.
Second regarding size limit that is entirely up to you. I would suggest nothing larger than an 8x10 photo (or other item) postage cost may be a concern for you. Be sure to include a SASE (Self Addressed Stamped Envelope).
Third I do not recommend requesting thru e-mail; I have done it with mixed success, but I believe that e-mail (disregarding "spam" factor) seems too impersonal to me. I believe that I can express myself better with pen and paper and it is more personal so the person you are writing to can hold your letter in their hand. Some may disagree with me regarding e-mail requests but, everyone has his/her own viewpoint.
