A long time before it starts. The earlier you arrive the better spots you will be able to stand.

When you arrive take a quick tour around all the places you could stand, and work out where would be the best place to be. It depends on every venue.

No as long as you don't keep her too long. Just ask for a quick picture then when she is about to move give it to her. If she is busy she might not be able to do both so make sure you have your letter ready to give in case she walks past.

Normally you are more likely to get a success by asking for an autograph. However, do what others are doing. If she's walking in a line taking pictures with everyone, take a picture. If she's only signing autographs then just ask for an autograph.
