here is an example:

Dear Lebra Loo

My name is John and I live in Manchester in the UK I love your acting! and hope that you continue to act in the future! I cant wait for your new show The chicken nugget show which I will definitely watch and maybe even buy on DVD! Anyway because you probably don't have much time because you are so busy filming the chicken nugget show I will get straight to it, Could you please sign the photo I have included/Could you please send me a signed photo of yours and sent it back in the SASE I have provided.

What to write on Envelope?

Shockingly I've never done this before so can someone help me? I'm from Australia and want to send a letter and photo to Ariana Grande for her to autograph and send back. So on the fan mail envelope I just write the address of the Fan Mail Club and her name, put the letter in and stick a stamp on the envelope and that's it? Then get another envelope and write my address and name and put in a photo that I want autographed and a couple of IRC's and put that inside the Fan Mail envelope too?

Also wondering if anyone has ever gotten anything back from her and if so what are the chances of It being an authentic signature? I remember in one of her videos she said she didn't like the fact that most celebrities used autopen and stuff but now she uses it too, but I understand that she has no time now so yeah.
