This really doesn't belong in feedback, but here is a bit of advice. If you are truly a fan seeking an autograph, you don't really need a template. Just write from the heart, tell them your a fan, tell them why, and tell them you would appreciate if they would sign the enclosed photo for you. It's not really a difficult task.
Don't worry about your English. There are millions of fans that don't speak or write English perfectly, and for a great many of them it's their first language.
Also, don't worry about your age. They don't know your age unless you tell them. In fact, I'd think they might be more inclined to sign if you were younger, being that your most likely not trying to make a profit off their signature.
