SENDING FAN MAIL FROM UK TO US

I have some questions about sending to US from UK:

1. how many stamps do I put on my SASE?
2. what are international reply coupons and do I need to include them (if so, how many?)
3. how many stamps do I put on my LOR's envelope?

sorry for all the questions. I've tried to find out myself, but with no success :)
