Rough outline...
Reply envelope, stamped with UK stamps for return postage (some say dollars work, some will accept it, some won't, stamps are better). Royalmail.com will tell you what value needs to be covered for the reply, depending on the weight of your package coming back.
The rest is the same as sending a letter within the states for you, insert in outer envelope, add items for signing, letter of request, and go for it.
