Keep track of your requests

Hi,
So many of us have problems keeping a track of what requests we have sent or when we sent it and many things else.
So sometime back I was using excel but once my laptop got virus and it got formatted.
So i found this on web to keep a record of my requests.

Its a type of log sheet. I just fill it and keep it in a file. Have fun using it. It will definitely make autograph collecting easier.
