It should be similar to your first one, but don't forget to put "c/o" (Care of)

Name OF Celebrity
c/o Irvine Bowl
650 Laguna Canyon Road
Laguna Canyon, CA 92651

If you want, you can put "Please hold for" before the name of the celebrity.
