That address is in the correct format. The suite always goes under the street address.

Yes, you would put "c/o" in front of "Evolution Talent Agency."

Can someone help me with how to write these addresses?

Allman Brothers Band
Evolution Talent Agency
1501 Broadway
Suite 1301 <------------shouldn't "Suite 1301" be up by Broadway?
New York, NY 10036
USA

and all the others that are like that? Or should it stay like that?

One more question,
Like the Allman Brothers Band example, for Evolution Talent Agency, would you put a c/o in front of it?
