I keep a binder organized by month and year. I put the date sent, date received, name of person, address used and if I used an SASE, SAE or just sent an LOR. I also note if I send a photo and if I got the photo back or not. I usually hand write it and then when the month is over I type it into an excel sheet and print it out so it looks neater.
I also have an acid free leather binder that I keep all my successes in and in that binder I have a log where I check in autographs. I have the person's name, date received, type (Ap, PP or authentic), category (music, actor, actress etc.) and if it was personalized or not.

I know I'm a bit of an organizational freak when it comes to this.
