I did some research and made this for the premiership.
Arsenal: Francesc Fabregas + Manuel Almunia + Robin Van Persie (rare) + Theo Walcott (rare) sign, the rest don't sign
Aston Villa: Most players do sign, some of the substitutes don't, they also send a factfile sheet
Blackburn Rovers: Nearly every player signs and even pays postage, they do not answer questionaires
Bolton Wanderers: They don't sign to my knowledge, I think there has only been a few successes from them in the past
Chelsea: Most of the players and the managers sign, they do not send their own pictures though
Everton: Very hit and miss. Sometimes send PPs, sometimes send authentics.
Fulham: They don't sign. They do send preprints via email though, of the whole team.
Hull City: Most of the players do sign. They can take some time though.
Liverpool: Steven Gerrard + Rafael Benitez are the only signers. For the rest, they send preprints via email.
Manchester City: Robinho + Richards + Wright-Phillips sign. Most of the others don't.
Manchester United: They all sign and even pay postage.
Middlesbrough: Hit and miss. Afonso Alves is a good signer.
Newcastle United: They all sign, but you have to pay postage by sending an SASE.
Portsmouth: Hit and miss. They do take alot of time to reply though.
Stoke City: They all sign, but don't send the pictures.
Sunderland: Hit and miss. Not many feedbacks to make a verdict on.
Tottenham: Preprints via email. Either SECs or authentics via TTM (hit and miss).
West Brom: Don't sign to my knowledge.
West Ham: Gianfranco Zola known to send stamps. Most are hit and miss.
Wigan: Most of the players do sign.
