ALWAYS! It's as easy to make 4 cards as one. I send at least 2 - one for the player one for me. Or sometimes 3-4 suggesting to the player they can keep the rest. If I don't have a picture of a player to make a custom card, I ask them to sign a team logo card (though I'm starting to customize those now too). Occasionally the player will send me a photo of themselves when they return my card. When this happens, I use it to make a custom card and and a bunch to the player, asking them to sign one for me and keep the rest. Had nothing but nice feedback from the players (especially those who only play 1 or 2 games).

I make custom photos for actors, other sports people (tennis, basketball etc) or musicians. Usually 15x20cm as they are perfect for framing and a good size for signing & posting, though 10x15cm are cheaper to post.

Printing custom photos is cheap too.

I also like to make custom bookplates for my books.

I do. I use Macromedia Fireworks which is a very professional program. The photos I make always turn out really nicely.
