I have a friend in Japan, so I'm able to tell you, that, generally speaking, this address is correct.
Of course, on the envelope it should be written as:

Horipro, Tatsuya Fujiwara
1-1-3, Shimomeguro
Meguro-ku,
Tokyo 153-0064
Japan

How it works?
1-1-3 - home/flat no.
Shimomeguro - street
Meguro-ku - a town where she/he lives
Tokyo - main city in the nearest area, where is main post office for this town
153-0064 - zip code (post code)

i found this address for an actor i want to send to, but...it seems diffrent from other japanese addresses ive sent to. it seems as though the person broke address down to much. can somebody fix it and put everything back in place? thanks!
