Many times, they have assistants to take care of their mail. (Think about the hundreds of letters they may receive a week.) The assistant many flag a letter for the celebrity to read, and sometimes the celebrity will answer letters, but most times it's putting a note on a photo saying who it's for. Then the stack is handed to the assistant, who mails the photos.
Their main job is to act, or sing, or whatever. That's their livelihood, with little time for anything else. Autographs are a privilege, not a right. And before anyone thinks or says they owe fans an autograph, without fans they'd be nothing, when's the last time a celebrity said, "I answered 500 autograph requests this week, you the studio should pay me for?" They'll still have fans regardless of whether or not they sign autographs (and not every fan who watches asks for an autograph) - even if people did stop watching their show or attending their concert, a whole lot more people would have to do so before the studio takes notice.
