I emailed some sports teams and requested fan packs yesterday, and these are the ones that replied today and their responses. Hope they're helpful!
MLB
-I emailed the Dodgers thru
CustomerService@ladodgers.com, and was told:
"Due to the number of requests we receive for complimentary items and merchandise, we unable to accommodate your request."
-I emailed the Mets thru
customer_relations@NYMets.com, and they said:
"Thank you for your email. We will be happy to mail you out one of our
2005 NY Mets Fan Packs."
-I emailed the Twins thru
Twins@twinsbaseball.com, and they said:
"Thanks for the e-mail. To receive a Minnesota Twins Fan Pack, please
send a request via U.S. Mail to:
Minnesota Twins
ATTN: TWINS FAN PACK
34 Kirby Puckett Place
Minneapolis, MN 55415-1523
A postcard or short letter will be fine. Sorry, the Twins do not
accept Fan Pack requests thru e-mail."
-I emailed the Royals thru
Chris.Lee@kcroyals.com, and he said:
"Thanks for your interest in the Royals and our 2005 Pocket schedule. Please complete the Pocket Schedule Request form and we would be happy to send you schedule and Fan Pack:
http://kansascity.royals.mlb.com/NASApp ... hedule.jsp "
-I emailed the Padres thru
comments@padres.com, and I got a really long email, but basically they told me I could get a fan pack if I donated at least $18 US to a charity they had, they'd send me a fan pack.
NBA
-I emailed the Heat thru
Guestservices@heat.com, and was told:
"Unfortunately, fan packs are no longer available for residents located outside the of the continental United States."
So...that's just more for people who don't live in the States!!
NFL
-And I emailed the Eagles thru
fanfeedback@lincolnfinancialfield.com, and they basically said YES! We'll send you a fan pack.

Sweet.
Hope that helped...'cause I know how helpful I can be!!
