This was from way back in June, but thought I'd report it in hopes of finding a new address that works.
On 6/9/15, I sent a LOR, one picture & $25 cashier's check (as requested on her site) with SASE to:

A couple weeks later I received my envelope back unopened & marked "unclaimed."
