On 7/18/2013 I sent former NFL QB Don Majkowski a LOR, SASE and 8x10 photo. I received it back today signed! With a nice Packers HOF 2005 inscription. I sent it to:

I got the address by emailing his website and asking how I could get his autograph and they told me I could send whatever I wanted and he would be more than happy to sign.
