Per Lefty's website, you can only request two sigs a year.

"Phil sends photos out free of charge in an effort to reduce the black-market value of his signature. If you have any other item that you would like to have him sign please send it to the address listed above. Phil will only sign pictures, flags, magazines, scorecards, and books. Please do not send hats, putter covers, trading cards or golf balls. Include only one item per request along with a pre-paid postage return envelope for your item and your contact information.
