I got this a week or so back. Used the EUE/Screen Gems Wilmington address for "Tammy".
Sent: May 17, 2013
Rcvd: May 24, 2013
Sent LOR, SASE, and 4x6 photo to the address. Received SASE back with signed and personalized pic.

shoot I put north before 23rd st not after. I hope it still gets to them.
UPDATED: My mom fixed it before it was mailed.
