What do I need to say on my letter?

I am wondering what I need to put on my letter that I send to an athlete for them to sign a jersey? I also want it personalized. Do I need to tell him that I'd like it signed on the back numbers?

Yeah I think you should tell him were u want it signed and personalized and all.
BTW the only thing I would be worried about is if you did not get your jersey back or if it does come back it might not be signed, just a little heads up. {up}
