It depends :)
Anyone asks about sth different. It depends what do you want to know about this person:) For example: about him/fer favourtite movie/color/song etc.

Hello :) ,

There are so many questions you can ask them. But remember that some questions are too personal. Like Ela and Paigey said you can ask them about their favorite book, movie, their hobby or anything else.

I've asked general questions like this to actors:

Who is your favorite person to work with?
Who is your biggest inspiration and why?
Why did you want to play (character)?
If that person is on a TV show, I may ask - What's your favorite storyline?

On the questionaire at the top in the center I type - "(Person's Name) Questionaire"

I also leave enough space between questions so they can write and at the bottom I write "Thank You!! :)"

And I aslo type the questionaire and print it on good quality paper (i.e. resume paper)

Honestly, I just ask whatever I want to know about the celebrity. I usually base the questions around who the celebrity is, and it's usually in this form:

Full Name:
Birthdate:
Hometown:
Favorite Color:
Favorite Food:
Best Friend:

1. Question I want to know.

2. Question I want to know.

3. Question I want to know.

This or That (based on the celebrity)
This or That
This or That

That's actually a pretty long questionaire, and I never make them longer than that, usually it's shorter, actually. So, don't make them too long, because the celebrity doesn't have time to answer a huge questionaire.
