Honestly I sent mine a little later, so I can't help you there but this is the address I used.

thanks for the adress but i figure if i send june 30 the letter will take a day or two to get their and by july fifth they will hold it or maybe i should send july 1 or 2 i will decide when it gets closer

I think the date you want to send it off is fine, they will hold it for them!!!
