Usually I include:
My name
What I admire most about the person.
What movies he/she has been in that I liked
Or if he/she is a singer, what songs I like the best
My address
Hope I helped a little. Good luck with whoever you're sending to!

TIP:
Personally, I noticed that when the subject line reads "PLEASE ADVISE" instead of "Autograph Request" you get a faster response. Thats from my experience with email
