My 1st Time Sending Out: Help!

This is going to be my 1st time sending out. I need some help.
I think that you need to send a SASE, something to be signed, and a letter. Correct? Am I missing anything?
2. I'm going to be sending out to the addresses below. Have you had successes or failures using these? For Peyton, should I send to the Peyback foundation and include a donation?
