When people mean letter, do they mean they put the letter in a big folder or did they just sent a letter?

If you are simply sending a letter to a celebrity with no intent of recieving anything back; you can just send it in a normal small envelope.
If you are sending a letter and a photo which you want signed, you need to send it in a big A4 or foolscap size envelope.
If you are sending a letter, with the intent of recieving the celebrities own photo, you need to send a large sized envelope too.
It all depends on what you want back, if anything..
