Quite a few people on fanmail.biz have had successes from concert venues. Check the Via Venue Feedback sub-forum to see which venues do and don't pass on autograph requests.

Basically, a normal fan letter, requesting your items to be signed.

Including some form of return postage is essential for a via venue request. You'd make it much more convenient and likely for the band/artist to reply if you included the relevant amount of stamps (from the country they're performing in), international reply coupons, or money (again, in the currency of the country - no coins!).
do you send photos or old albums ? are there good chances of getting stuff signed via concert venue ?
You can send any item you'd like to be signed. Photo's are the easiest choice - they're cheap, save on postage, and you won't be devastated if they're not returned to you. The chance of getting your stuff signed via venue depends on the person(s) and venue you're writing to. It's risky, and doesn't always work - but worth trying to get that rare or personalized autograph.
how much time do you allow for the band to arrive to venue ?
If you're sending to a venue from overseas, mail your request at least a week and a half to two weeks in advance. If you're within the country of the concert, you can send it a few days before the performance.
