When writing to a celebrity

Ok so I'm starting to write to some celebs and I'm struggling on how I put in my letter a request for a autograph as I don't want to offend the celeb in anyway.

I also am unsure about this I heard some celebs send out there own pictures as i want a decent picture but have searched for hours and have had no luck.
