I just need help with two phrases in my letter of request please. Is it correct for me as a fan, to say 'Liebe Herr' (Dear Sir)? I keep thinking the word 'Liebe' has a sort of close-relationship/love connotation because of its use in 'ich liebe dich', and I don't want to address the people I'm writing to inappropriately. Also, how do I say 'I hope you are well/doing fine' - is it 'Ich hoffe, das Sie in ordnung sind' or 'Ich hoffe, dass Sie sehr gut sind'?
Any help greatly appreciated, thanks ;)

"Lieber Herr" is correct and it's usual phrase in letters, you can use it, it has the same meaning as "Dear Mr / Sir" 
As for your second question, I'd say "Ich hoffe, dass Sie OK sind."
But I'm not german, I've just learnt german for sevetral years. I hope I helped you.
