the questionnaire is cool. ive never sent one, but may start.
do you send one with every request?
how have the responses been?
is each one specific to who you are sending it to or do you have a standard one you send to everyone?

No, not with every request. Usually I don't send one to people I think are really big stars and won't have any time (e.g. Christian Bale, Angela Lansbury, Steve Carrell, etc.), or are pretty old or in poor health. For the most part I just send them to Star Wars related people too. Although I have sent to a few other stars who have responded.
Success rates are pretty good - I would estimate that about 75% fill it out and 25% don't return it.
I do use templates I've created for people in terms of what their profession is. I have a questionnaire that I send to artists, one for authors, one for crew members, one for voice actors, and one for general actors, although I do try to change up a question or two for every person.
Here's a few people I have had reply to them:
Pretty much all the Clone Wars voice cast will, except Ashley Eckstein didn't for me.
(James Arnold Taylor, David Acord, Matt Lanter, Nika Futterman, Tom Kane, Ian Abercrombie, Matt Wood)
Crew:
(Peter Kuran, Mark Sullivan, Tom Martinek, Russell G. Chong, David Sosalla, Pete Romano, Alex Jaeger, etc.)
Actors:
Mark Capri, Al Lampert, Vic Armstrong)
Steve Sansweet and Dave Filoni told me they were sorry but didn't have time.
I only started doing questionnaires like 6 months ago, so there are a ton of other people I've already received that will probably fill one out. Just ask yourself "Is this person probably getting flooded with mail right now?" And if you answer "no", then you're probably good to go. {thumb2}

I send out Q&A cards with many of my requests. I'm never sure who will answer them, but it's always a real joy to see the ones that they do fill out. I've gotten 15 so far and numerous handwritten letters as well. {thumb2}

Today
Paul Weston (I sent it to 'The Spotlight' address, and they redirected it to the 'Old Oak Barn' address. Then I got it RTS from them saying 'addressee has gone away')
