C/O (sometimes written in lowercase as c/o) means "care of." In a nutshell, it lets the post office know exactly where you are trying to send your mail to. If an error is made in the address, the post office will know you are trying to send your mail to the baseball team's stadium and can make corrections to insure the mail gets there. Omitting the "C/O" could make the post office think you are looking for a specific person, and could result in an "Return To Sender, Address Not Found" result, since it is not likely the player will have a published address in the same city.
