Former MLB/Current Coach Jacob Cruz Address Request

I'm trying to find a valid address for former MLB outfielder and coach Jacob Cruz. I'd also like to hear about any feedback from anyone who has tried sending a request TTM to him.
Born January 28, 1973. He played professionally from 1996-2009 for the SF Giants, Cleveland Indians, Colorado Rockies, Detroit Tigers, Cincinnati Reds, Hanwha Eagles, Samsung Lions, and La New Bears. He was the assistant hitting coach for the Pittsburgh Pirates in 2019 and the Milwaukee Brewers from 2020-2021. He currently is the assistant hitting coordinator for the San Francisco Giants. I might try sending to the Giants address but if anyone had had success sending to a different address please let me know. Thanks.
