I want to send something to Jim Thome and I didn't know if he would sign my Phillies poster even if he doesn't play for them should I include a index card encase?

Yes they do. I've had lots and lots of success through sending stuff to pro athletes, not just MLB players.

I don't believe Thome is that way.

The only one I knew of was Dave Stewart. (No Rangers stuff)

Thats correct, Jim Thome is extremely nice. Doesn't sign much TTM anymore though...but I don't think he'd have an issue signing anything, no matter what team he was playing for on the card/poster.
