Hi! I usually write Mr. Craig, Mr. Cocker, Ms. Slick, as it is more respectful and formal. As for the honorific addresses, they must always be used along with the birth name or the full name (e.g. Sir James Paul McCartney, Sir Paul McCartney, or Sir Paul, but never Sir McCartney), according to the formal protocol.
